Care plans, NDSS & coordinated care

If you have diabetes, your GP can prepare a chronic condition management plan to coordinate your care and, where eligible, help you access Medicare-subsidised allied health (such as a dietitian or podiatrist). We can also help you register with the National Diabetes Services Scheme (NDSS), which provides subsidised diabetes products and information.
