At Marketplace Medical & Skin Clinic, our GPs provide health assessments and care plans that help identify risks early and support the ongoing management of long-term conditions. For eligible Medicare patients, these are bulk billed – with no out-of-pocket cost.
Preventative care to help you stay well
A health assessment is a structured check-up that looks at your overall health, identifies risk factors, and helps you and your GP plan ahead. Care plans help coordinate care if you’re living with a long-term condition. Both are part of a preventative approach – focusing on early detection and ongoing support rather than waiting for problems to develop.
Health assessments we offer
45–49 year old health assessment:
A one-off health check for people aged 45–49 who are at risk of developing a chronic disease – a chance to review your health and act early.
75 years and over health assessment:
A comprehensive health check for people aged 75 and over, reviewing your physical and general wellbeing and any support you may need.
Heart health check:
A check of your heart-disease risk factors, including blood pressure and cardiovascular risk.
Skin cancer check:
A check for signs of skin cancer as part of your preventative care, important in sunny Queensland.
Care plans for ongoing conditions
Chronic condition management plan:
If you’re living with a long-term condition (such as diabetes, heart disease, asthma or arthritis), your GP can prepare a management plan that sets out your care goals and coordinates support – and, where appropriate, helps you access allied health services.
Mental health care plan:
A mental health care plan helps structure support for your mental wellbeing and can help you access sessions with a psychologist or other mental health professional.

Do I need to do anything to prepare?
Just book an appointment and let reception know it’s for a health assessment or care plan, so we can allow enough time. Bring a list of your medications and any health concerns you’d like to discuss.
How often can I have a care plan reviewed?
Care plans are reviewed regularly to track your progress and adjust your care. Your GP will advise how often a review is recommended for your situation.
