A health assessment is a structured check-up that looks at your overall health, identifies risk factors, and helps you and your GP plan ahead. Care plans help coordinate care if you’re living with a long-term condition. Both are part of a preventative approach – focusing on early detection and ongoing support rather than waiting for problems to develop.
A one-off health check for people aged 45–49 who are at risk of developing a chronic disease – a chance to review your health and act early.
A comprehensive health check for people aged 75 and over, reviewing your physical and general wellbeing and any support you may need.
A check of your heart-disease risk factors, including blood pressure and cardiovascular risk.
A check for signs of skin cancer as part of your preventative care, important in sunny Queensland.
If you’re living with a long-term condition (such as diabetes, heart disease, asthma or arthritis), your GP can prepare a management plan that sets out your care goals and coordinates support – and, where appropriate, helps you access allied health services.
A mental health care plan helps structure support for your mental wellbeing and can help you access sessions with a psychologist or other mental health professional.
